Easy Onboarding Process and Unattended Data Collection

GCARE patient-facing and caregiver applications can be installed on iOS and Android smartphones. Our mobile apps have a clean and intuitive interface and facilitate seamless connectivity with a vast number of medical sensors.


For an even smoother onboarding process One Click Login can be enabled: patients simply click on the link provided by their caregiver (via email or text).


Unattended data collection via GCARE-powered hubs can be used to receive data in the background. This solution specifically targets care coordinators and nurses monitoring elderly persons.

Caregiver Portal and Patient-Caregiver Communication

Caregiver Portal is an integral part of the GCARE RPM Platform. GCARE Server software is available in both in-cloud and hosted editions and ready for HIPAA and GDPR compliant deployments. The portal enables web-based access to collected measurements and historic data views, trend graphs, and reports.


GCARE RPM platform has built-in reminders to ensure that patients take their measurements and medicine on time. Thresholds can be automatically set to alert caregivers via SMS (text messages), push, or email when a patient’s reading falls above or below the set norm.

On request, in-calling option can be activated inside the patient-facing application. This can be done via CallShaper or another tool that the caregiver is most familiar and comfortable with.

Care Circles and Export to EHR

Aside from sharing their medical readings with healthcare providers, patients can use the GCARE-powered portal for creating their own Care Circles and granting the right of data access to their loved ones. This feature is highly appreciated by end-users, but also helps caregivers to grow their reach, get referrals, and onboard new patients.

Rapid integration with third-party EHR solutions is possible. GCARE uses a proprietary API for exchanging data with third-party electronic health record systems, but the Fast Healthcare Interoperability Resource protocol - commonly known as FHIR – can also be used for data exchange.

Remote Physiologic Monitoring CPT Billing Codes

Globally, healthcare providers are turning to remote patient monitoring software to cope with the Covid-19 pandemic and to improve the management of chronic illnesses, reduce hospital stays, and improve patient-doctor collaboration. In the USA, four CPT codes published by the American Medical Association in 2020, are associated with Remote Patient Monitoring services offered by healthcare providers to patients:

  • CPT code 99457 - remote monitoring of patient data for 20 minutes per month, can be billed once every month. “Interactive communication” can include both in-person and connected health channels.

  • CPT code 99458 – an additional 20 minutes of monitoring on top of the requisite 20 minutes (code 99457) per month.

  • CPT code 99454 - providing patient with an RPM device for a 30-day period, can be billed once every month.

  • CPT code 99453 - setup & patient education on use of devices. Can be billed one time.
